Calcium Alpha-Ketoglutarate (CA-AKG): What the Biological Age Study Actually Showed

The CA-AKG biological-age study is widely quoted and widely misread. Review the mouse evidence, uncontrolled human pilot, and unanswered questions.

You've seen the headline: CA-AKG reversed biological age by eight years.

The study is real. The number is real. And almost every page repeating it leaves out the detail that determines how much weight it should carry.

Let's go through it properly, because a compound with a genuinely interesting mechanism is being undersold by overselling.

What Alpha-Ketoglutarate Does

AKG sits at the centre of the citric acid cycle — the sequence of reactions inside mitochondria that extracts energy from fuel. It's a metabolic hub, not a peripheral molecule.

It has roles beyond energy. AKG is the obligatory co-substrate for a family of dioxygenase enzymes including the TET enzymes, which mediate DNA demethylation, and the JmjC histone demethylases. These are epigenetic regulators — proteins that determine which genes are read in which cells. That's the mechanistic basis for the interest in AKG and biological ageing, and it's a legitimate one. Circulating AKG declines substantially with age. One frequently cited analysis found a roughly tenfold decrease between ages 40 and 80.

The Animal Evidence

Asadi Shahmirzadi and colleagues (2020), in Cell Metabolism, gave calcium AKG to mice starting in mid-life. Treated animals showed reduced frailty and a modest extension of lifespan — but the more striking result was the effect on healthspan: the treated mice spent a substantially larger proportion of their remaining life in good health, compressing the period of decline at the end. That healthspan-versus-lifespan distinction is the genuinely interesting finding, and it's the one that gets dropped in favour of the sexier number.

Now the Human Study — Read Carefully

Demidenko and colleagues (2021), in Aging, reported on 42 people taking a calcium-AKG-based formulation, with DNA methylation age assessed using the TruMe methylation clock. Average methylation age was reported approximately eight years lower than at baseline. Human, uncontrolled

Here is what that study was not:

  • There was no placebo group. No control arm means no way to separate the intervention's effect from regression to the mean, seasonal variation, or the lifestyle changes people typically make when they start a longevity protocol and know they're being measured.
  • It was not blinded. Participants knew what they were taking.
  • The sample was 42 people, self-selected — the kind of person who buys a longevity supplement and pays for methylation testing is not a random sample of anyone.
  • The formulation contained other ingredients. Attribution to AKG alone isn't clean.
  • Methylation clocks have real measurement variance. Different clocks disagree with each other on the same sample, and test-retest variability on some is substantial.

None of this makes the result worthless. It makes it a pilot study generating a hypothesis worth testing properly — which is exactly how the authors framed it, and exactly how the marketing does not.

A randomised, placebo-controlled trial is needed. A 2022 review noted that recent human studies had not demonstrated an anti-aging role for AKG and called for further clinical research. Until controlled results are published, the methylation finding remains hypothesis-generating rather than evidence that CA-AKG reverses biological age.

Why the Calcium Salt Form

Free alpha-ketoglutaric acid is unstable and degrades quickly in the acidic environment of the stomach. Binding it to calcium produces a stable salt with a slower release profile, surviving gastric transit for absorption further along. This is also the form used in the published research — CA-AKG, not free AKG.

Note that CA-AKG contributes meaningful elemental calcium at typical doses. Factor that into your total intake if you already supplement calcium.
